CommuniCare Recognized as a Top Performer in Clinical Quality

In May 2019, Partnership HealthPlan of California released results from its Quality Incentive Program, which rewards its contracted sites for clinical quality.

CommuniCare Health Centers was awarded the highest quality incentive payment in its history and was recognized as the top Yolo County healthcare organization and among the top five of 123 healthcare organizations throughout the Partnership HealthPlan Region.

About CommuniCare Health Centers

CommuniCare Health Centers is a Federally Qualified Health Center providing care to one in eight Yolo County residents.  CommuniCare offers comprehensive primary medical and dental services, perinatal services, behavioral health services, substance use treatment, health education and outreach services to the low-income, diverse, uninsured and Medi-Cal populations of Yolo County and eastern Solano County, including migrant and seasonal farm workers and their families. CommuniCare has three primary care health centers and an additional ten satellite sites, with locations in Davis, West Sacramento, Woodland, Esparto, Winters, and Dixon.
