Preparing For Your Visit
Preparing for your visit will help your care team provide you with the best possible care. For every appointment, please bring the following items:
• Picture ID
• Insurance card, if you have one
• Any medications you’re currently taking
• For children’s appointments, bring your child’s immunization records
• A list of questions or concerns you would like to discuss with your care team

Q: Can I make an appointment online? Can I cancel or change an appointment online?
A: Currently, online scheduling is not available. Please call the health center directly to make or cancel an appointment.
Q: Can you help me get health insurance?
A: Yes, our Certified Enrollment counselors can help you apply for health insurance or programs such as Medi-Cal, Covered California, County Medical Services Program (CMSP), and CalFresh. Call us today to make an appointment with a Certified Enrollment Counselor.
Q: What is the cost of a medical visit?
A: CommuniCare accepts most health insurance plans and coverage including (but not limited to) Medi-Cal, Partnership Health Plan, Medicare, County Medical Services Program (CMSP), Blue Shield and Anthem Blue Cross. Sliding Fee Discounts are available to all patients who meet family size and income requirements.
Q: Do you accept walk-ins?
A: CommuniCare accepts walk-ins. However, wait times vary and there is no certainty that you will be seen the same day. It’s highly recommended that you call to schedule an appointment with the health center to receive faster service. We often take same day appointments.
Q: What are my payment options?
A: We accept cash, check and credit card. Payment plans are available. Ask the Clinic Reception staff for more information.
