CommuniCare’s Perinatal Behavioral Health Services are available to patients who desire pregnancy and postpartum wellness care, even if you are not receiving your pregnancy care through our Perinatal Department.

Programs Offered

Maternal Mental Health – In Home Therapy for Caregivers

In-home, in-clinic, or telehealth support is available for persons experiencing depression, anxiety or other emotional symptoms during pregnancy or up to five years after giving birth. The program includes 10-15 sessions with a behavioral health clinician, and you do not need to receive primary or perinatal medical care through CommuniCare or carry health insurance.  Maternal mental health services are free of charge.

The Child Project – Road to Resilience Program (R2R)

In collaboration with First 5 Yolo and the Yolo County Children’s Alliance, CommuniCare offers free support services to pregnant persons and families with infants under 12 months old.  Through regular visits, families receive assistance with navigating complicated service systems so they can focus attention on their new baby.  Participating families receive compensation for their time in the form of gift cards and other incentives.

Journey HOME (Healing through Outpatient treatment & Maternal Education)

Journey HOME is an outpatient program for women and children that includes individual and group therapy along with education and support for issues related to parenting, relationships, trauma-recovery, and relapse prevention.  The program includes free transportation to-and-from treatment is provided for women and children participating in the program.  Our Child Development Program (CDP) also provides free childcare for children whose mothers attend Journey HOME.

  • Outpatient therapy program for women and children
  • Access to substance use disorder treatment
