Adult Behavioral Health Services

CommuniCare Health Centers offers a range of Adult Behavioral Health services to support comprehensive mind and body wellness in our patients. CommuniCare patients can access our Integrated Behavioral Health services therapeutic services for children and adults as part of Primary Care at CommuniCare. Adult and Child Psychiatry is also available by referral. Patients who identify as Latino/Hispanic can access behavioral health services free of cost from bilingual/bicultural clinicians through the CREO program. Adult Behavioral Health services are offered at our locations in Davis, West Sacramento and Woodland and are also imbedded at the Yolo County Health and Human Services offices in West Sacramento and Woodland (for CalWORKS recipients).

IBH services are available for patients (ages 4 and up) referred by their primary care providers at CommuniCare Health Centers. Services include brief, confidential individual counseling services for children and adults, as well as access to adult and child Psychiatry.

Mental health navigation services are available at the Mental Health Navigation Center located at 600 A Street, Davis.  Staff are available to answer questions at (530) 204-9467. The Clinic is open Monday – Friday from 8:30 am – 5:00 pm.

Mental Health Navigation Center is partnership between Yolo County Health and Human Services Adult Mental Health and CommuniCare Health Centers and serves as an entry and access point for mental health and substance use disorder treatment services in Yolo County.  We provide the following:

  • Crisis Support
  • Mental Health Assessments, referrals, and linkage to care providers
  • Substance Use Disorder Screenings (ASAM) and linkage to treatment
  • Entry point for adult mental health services and medication management with Yolo County Health and Human Services Agency
  • Adult Mental Health Wellness and Social Support Groups
  • Telehealth and in-person care available
  • Case Management, Care Coordination, and Peer Support
  • Transportation
  • Help accessing community services and resources

CREO services are aimed at improving access and increasing life functioning for the Latino/Hispanic community of Yolo County struggling with behavioral health issues. CREO provides primary care and full-scope behavioral health services (counseling/psychiatry/substance abuse). All services are delivered in Spanish by clinicians who are culturally reflective of the community and offered no-cost to qualifying participants.

Families receiving CalWORKS benefits have access to no-cost individual, family, and couples counseling along with parenting support and substance abuse treatment. Counseling and case management services are embedded in the Department of Education and Social Services Office in both West Sacramento and Woodland.
