Substance Use Services

CommuniCare Health Centers offers Outpatient Substance Use Disorder Services in Woodland and West Sacramento. Services are available to Adults and Adolescents and CommuniCare offers specialized outpatient services for Dually Diagnosed Adults, Pregnant and/or Parenting Mothers as well as individuals who need medication assistance in stopping Opiate and Alcohol use. There are two ways to access services:

PNDT is an outpatient program for women and children. PNDT includes individual and group therapy along with parenting, relationships, trauma-recovery, and relapse prevention. A variety of evidenced-based and cognitive behavioral curriculum are offered. No-cost transportation to and from treatment is provided for women and children attending PNDT. No-cost child care is also offered in the Child Development Program (CDP) for children whose mothers attend PNDT.

Designed for men or women who are struggling to abstain from substances and need frequent supportive contacts throughout the week.  ASAM Level 2.1

Location:  Hansen Family Health Center, Woodland office

  • 3 hours a day/4 days a week (Monday, Wednesday, Thursday, Friday)
  • 9:15 am – 12:30 pm
  • Individual and group therapy
  • Evidenced based interventions including Seeking Safety, Dialectical Behavior Therapy (DBT), mindfulness/meditation practices, addressing relationships, trauma-recovery, drug education, employment issues and mental health care.

Outpatient Substance Abuse and Recovery Program (OSARP) ASAM level 1.0

Location:  Hansen Family Health Center, Woodland and Salud Clinic, West Sacramento

  • 3 Groups per week, weekly individual counseling
  • Weekly random testing
  • 5:00pm to 6:30pm on various days
  • Evidenced based interventions including Seeking Safety, Moral Reconation Therapy (MRT), mindfulness/meditation practices, addressing relationships, trauma-recovery, drug education, employment issues and mental health care.

Location:  Hansen Family Health Center, Woodland and Salud Clinic, West Sacramento

  • 3 Groups per week, weekly individual counseling
  • Weekly random testing
  • 1:00pm to 2:30pm on various days
  • Evidenced based interventions including Seeking Safety, Dialectical Behavior Therapy (DBT), mindfulness/meditation practices, addressing relationships, trauma-recovery, drug education, employment issues and mental health care.

SAE is offered in both English and Spanish and focuses on providing education about frequently abused substances, including impacts on health and mental health, as well as information about substance use disorders. This program meets the PC1000 requirements.

Spanish Substance Abuse Education ASAM level 1.0

Location:  Hansen Family Health Center, Woodland

  • 1 Group per week and weekly individual counseling
  • Weekly random testing
  • 5:00pm to 6:30pm on various days
  • Evidenced based interventions including Seven Challenges, Dialectical Behavior Therapy (DBT), mindfulness/meditation practices, addressing relationships, trauma-recovery, drug education, employment issues and mental health care.

Medications (Vivitrol and Suboxone) are offered for treatment of opioid dependence for our medical patients and are prescribed as part of a comprehensive Medication Assisted Treatment (MAT) plan that includes counseling and participation in social support programs. Clients enrolled in MAT must attend individualized counseling services and outpatient programming.

Young Adult Group ASAM level 1.0

  • 2-3 Groups per week, weekly individual counseling
  • Weekly random testing
  • 5:00pm to 6:30pm on various days
  • Evidenced based interventions including Seven Challenges, Dialectical Behavior Therapy (DBT), mindfulness/meditation practices, addressing relationships, trauma-recovery, drug education, employment issues and mental health care.

CBI-SA teaches youth strategies for avoiding substance abuse and emphasizes skill building activities for promoting cognitive, social, emotional and coping skills development.
