Our Clinic-On-Wheels provides care to the most vulnerable patients in Yolo County .
CommuniCare’s Mobile Medicine team is on the road every week to meet you where you are and make it easier to obtain immediate, on-site medical and mental health care service without an advance appointment. For non-urgent concerns – such as a medication refill, to speak with an advice nurse, or to request a visit from the Mobile Medicine unit – please call the Mobile Medicine Message Line at (530) 758-1510, ext. 1234.
About Mobile Medicine
CommuniCare’s Mobile Medicine Team is on the road every week reaching out to our most vulnerable patients in Yolo County, the migrant workers and the uninsured and unhoused population. The team offers preventive screenings, wound care, mental health services, dental care, health education, referral to social services and primary care follow up.
Services-include:
- Wound care
- Blood pressure checks
- Infection treatment
- Diabetes support
- Ordering new medications, transferring care details
- COVID-19 symptoms
- Behavioral health, stress, depression, anxiety, substance use
- Guidance in relation to housing, social security and government assistance
