A patient portal is a secure online website that gives patient convenient, 24-hour access to personal health information from anywhere with an internet connection. Using a secure username and password, patients can view their health information.

Preparing For Your Visit

Preparing for your visit will help your care team provide you with the best possible care. For every appointment, please bring the following items:

 

• Picture ID
• Insurance card, if you have one
• Any medications you’re currently taking
• For children’s appointments, bring your child’s immunization records
• A list of questions or concerns you would like to discuss with your care team

CommuniCare accepts most health insurance plans and coverage including (but not limited to) Medi-Cal, Partnership Health Plan, Medicare, County Medical Services Program (CMSP), Nivano, and River City Physician’s Group. Our trained insurance specialists can help you with the Medi-Cal application process.   If you are already enrolled in a Medi-Cal plan, we’ll assist you with the necessary forms or steps to enroll in our care programs.

You are not required to have health insurance to receive services at CommuniCare.  Sliding Fee Discounts are available to all patients who meet family size and income requirements.

Our Client Benefits Advocates can help you apply for health insurance or programs such as Medi-Cal, Path to Health, Connect to Care, County Medical Services Program (CMSP), and CalFresh. Call one of our clinics and ask to make an appointment with a Client Benefits Advocate.”

Yes, CommuniCare Health Centers has a translation service on hand. Please mention which language you would like when you make your appointment to help us prepare in advance!
