We receive funding through a variety of sources, but not enough to meet the need. That’s why your generosity is critically important to our mission, our patients, and our communities.

As healthcare costs continue to rise, seeking healthcare is simply out of reach for far too many of our neighbors. For the nearly 6,000 patients without health insurance who rely on CommuniCare each year, this is certainly the case.

 

That’s why CommuniCare is here. To be the safety net. The lifeline. At CommuniCare Health Centers, everyone who walks through our doors is provided a full range of primary care, dental, and behavioral health services regardless of their ability to pay.

Mobile Medicine

Mobile Medicine

Serves unhoused individuals and seasonal agricultural workers. The van operates 12 hours a week rotating between Davis, Woodland, West Sacramento, and the Capay Valley.
Food as Medicine

Food as Medicine

Empowers patients to grow, access, prepare, and enjoy locally grown food for holistic well-being via a quarter-acre clinic- based garden and food education.
Smile Savers

Smile Savers

A voluntary, school based, dental disease prevention, and oral health education outreach program. The Smile Savers Program serves elementary and pre-school aged children in schools, pre-schools, and special education programs throughout Yolo County.

Perinatal Program

Perinatal Program

Includes comprehensive services for prenatal and postpartum care. Provides free of charge car seats, baby carriers, and wraps to new parents.

Elevate Queer Yolo

Elevate Queer Yolo

A free program empowering and celebrating queer youth and young adults ages 12-26 in Yolo County. Offering mental health, peer mentorship, social support groups, and primary care services.
