Community Programs for Individual Behavioral Health
Los servicios de CREO tienen como objetivo mejorar el acceso y aumentar el funcionamiento de la vida de la comunidad latina/hispana del condado de Yolo que lucha contra los problemas de salud del comportamiento. CREO proporciona atención primaria y servicios de salud conductual completos (asesoramiento/psiquiatría/abuso de sustancias). Todos los servicios se prestan en español por médicos que reflejan la cultura de la comunidad y se ofrecen sin costo alguno para los participantes que califican. Transitions of Care (TOC) provides case management services for Yolo County community members experiencing chronic physical and/or mental health challenges. Support includes connections to ongoing mental health, substance use and physical health services, and case management.
Mental Health Diversion is a new program, created in partnership with the Yolo County District Attorney’s Office, Public Defender’s Office, Probation, and Yolo County HHSA. The goal is to divert individuals experiencing a mental health and/or substance use disorder from the revolving door of incarceration through clinical treatments, case management, peer support, and connection to stabilizing resources in the community.
