Yolo County is asking for the public to complete a Community Health Status Survey in order to identify the most important health concerns and barriers to health and well being for the residents of Yolo County. As part of the County’s Community Health Needs Assessment process, this survey is done every three years. The survey is anonymous and should be completed only once and is only for Yolo County residents.
Responses must be obtained by August 25th.
Participants will have the option to enter for a chance to win a $100 grocery gift card! The survey takes 5 – 10 minutes to complete.
