CommuniCare Health Centers’ mission is rooted in the underlying philosophy that health care is a right, not a privilege. Our mission is characterized by the following commitments:

  • We provide comprehensive and high-quality health care to anyone in our community, regardless of insurance status or ability to pay.
  • We deliver health care in a supportive, safe environment emphasizing patient partnership, wellness and preventive health in all of our services.
  • We invest in employees, learners and volunteers at all levels of the organization, as they are the foundation and heart of the work we do.

Providing Comprehensive and
High-Quality Care Since 1972

Using the patient-centered medical home model of care, CCHC puts patients first by providing comprehensive, evidence-based, quality health care services delivered by a dedicated team of providers and support staff. Land Acknowledgement

Davis and Woodland

We would like to acknowledge the land on which our health centers are built and on which we live, work and serve our community. For thousands of years, this land has been the home of Patwin people. The Patwin people have remained committed to the stewardship of this land for many centuries. Much Yolo County land has also been shared by the Miwok people. We are honored and grateful to be here today on their traditional lands.

We acknowledge the Nisenan people are still here among us today, though they are nearly invisible and their story continues to be excluded from history. We understand we are on Nisenan land that was never ceded and the original tribal families have yet to recover from the near genocide of their people. As residents and visitors in Nisenan land, we stand beside the Nisenan people and their right to sovereignty, self-governance and self-determination through the stabilization of their people, homelands and culture.
