Our informed providers with clinical expertise in LGBTQ+ specific topics work together with you, your partners(s), and your chosen family to manage your personalized care plan. Through the combination of specialty services, behavioral health care, individualized care plans, and referrals, CommuniCare takes a holistic and inclusive approach to your care needs.
Affirming Services For Yolo County's LGBTQ+ Community
Each of our clinic offers inclusive services by LGBTQ-informed providers, such as:
- Medical, dental, and prenatal care
- Behavioral Health care (known as Integrated Behavioral Health)
- Substance use and recovery support
- Peer and social support groups, provided by Elevate Queer Yolo
- Gender-affirming medications (like testosterone, estradiol, spironolactone) for adults
- Letters for medical clearance
- Referrals to specialists, when indicated
- Assistance and advocacy with legal name and gender documents

- Personalized help with getting medical insurance, finding housing, food, and social support
Sexual Health support
- STI & Birth control methods at Safe Care Clinic
- Confidential charts (separate from general medical charts)
- PrEP & PEP (pre-exposure prophylaxis, post-exposure prophylaxis) prescriptions to reduce risk of potential HIV contraction or transmission
- HIV Prevention & Care, including Ryan White services

Visit our Safe Care Clinic for free, confidential, and private sexual and reproductive health services. Services include all forms of birth control, pregnancy testing, STI testing and treatment, HIV testing, and emergency contraception pills. All services are available for all patients ages 12 and older on a voluntary basis. We can also help connect patients to CommuniCare’s other health care services.
FAQ’s for LGBTQ+ Care
Hormonal therapy for adolescents is generally referred to specialty care settings outside of CommuniCare and always requires parental consent.
